Required Information

To complete the online application you will need the following information

• Current Address information
• Address history for the past 5 years

Once your application has been processsed we will require the following proofs from you

• 2 proofs of your current address (dated within the last six months)
• Photographic Identity
• Any relevant medical information
• Proof of National Insurance Number

Please note an application can take up to 10 working days to process.
